I am no expert, but have written 5 final reports (we call them control plans), this is what I do.

Front page is a sign off sheet for the project. Includes the names of all the team members, sponsor, champion/MBB, local finance & corporate finance.

Then is a short introduction, stating the problems and reason for the project and the goals of the project.

Then a description of the key 6s tools used and where used (definatley not too technical)

Next is the control plan i.e. how to monitor & measure the performance after you hand it back and what to do if the process is going out of control. Remember the sustain part of this is the most important.

Finally a financial summary and projected ROI/EVA for the project.

Also I usually stick in a couple of key charts as appendix. C&E metrix, I&MR charts showing performance before and after improvement phase.

In total the document is only 6 pages excluding appendix. Hope this helps / what you are looking for. I was in the same possition as you once and just had to do something, so far no one has complained.

The DOLLAR TEST

Hold a dollar at your shoulder, start your presentation and release the dollar. 

If the VP is not interested by the time the dollar hits the ground, you fail. 

Its all about the money. 

I feel thats all they want to see so that's all we need to spend our time showing them.  If they have questions on how the money was made or saved, go to slide two and begin the explanation.

I use a Power Point format and send the report out to our Upper Management Staff on a monthly basis. In this report (called "Six Sigma Metrics Report") I identify in the beginning the project and objectives, then I follow that with the team members and "Metric Owners" who contributed to the success of the project. After that I put in a few of the CTQ metrics and their current performance against baseline performance (basically I show a rolling time period to demonstrate continuous improvement). At the end of the report I include the Control Plan for the project along with the Project Financial Report (I put this at the end so that I know the management staff will have to look through the CTQ's before they get to the bottom line $$). I have had good response from the staff on this communication method.

Keeping the audience in mind (a Vice President with or without knowledge of Six Sigma methodology details), I would start with the DMAIC process/briefing you use to "certify" as a green/black belt and synthethize it to meet the audience's needs. I would start out with a context setting moment for the VP if he needs it and then launch into the report/presentation.  I would report in a "veni, vidi, vici" format ("I came, I saw, I conquered") making sure the VP got the message on the ROI and how you are now armed with tools to conquer other company process problems.

Hello folks,

Its report time!  I searched the iSixSigma site for past postings regarding project reports, only one posting about a year ago.  Some good ideas were found.  Does anyone have any good thoughts or formats that worked well for them to add that I can (steal)...I mean borrow?

Has an uniform accepted format been developed for 6S?  Seems something as powerful as 6S that uses the same tools across the boards would be reported out the same way so anyone would understand what was done.

Jeepers..I'm begining to worry about the Red Sox after this weekend too.

I've consulted for years now on Six Sigma and have yet to see the same reporting structure or reports used at different companies. Each company likes to "accomplish" things in different manners -- it speaks to the company culture. So, unfortunately, no one template fits all sizes.

I personally like the one page review sheet that includes a summary of each phase, main tools used, main findings. I can then ask BBs the significance of the tools/findings, etc. Usually the BB will need a few pages per phase as backup information during the tollgate reviews.

For us, the main purpose of a final report is to share the knowledge obtained during the development of your project and results (wether good or not good).  We have two ways of doing this: 

1.- Project posters, which are updated during the project development period and posted in different locations where many people can see them to promote conversation and obtain new ideas (this also helps to generate buy-in for your solutions and new projects).  

2.- Final project report, where we ask the Blackbelts to share experiences (good and bad, barriers, new tools they discover along the way, etc.), around the whole process along with their final results in terms of impacting a business KPI or financial return.  This report is then formally handed over to the process owner who takes responsibility for sustaning and reporting the benefits, and completing implementation of longer term solutions.  The report includes at least: Team Charter, Stakeholders and their Requirements, Data Collection and Analysis,  Baseline Sigma Calculation, Root Cause Analysis, Possible Solutions Identified and Evaluated, Selected Solution/s, Pilot (when applicable), Implementation Plan (including Gantt Chart, training and communication), KPI´s and Monitoring System (including responsible persons).  We also ask them to document all their team meetings with agendas and minutes to assess their facilitation and knowledge transfer skills.

We also use this report to identify any transferrable knowledge that can be applied in other parts of the business.

You are doing two things 1) Reporting your success ,2) Selling the six sigma methodology.  Both are important

Typical presentations in sixsigma are referred to as a storyboard, this document should be a cumulative document enlarging as you go through the various stages DMAIC of a process.  The key to a successful project is a demonstration ofthe 1) Problem statement, 2) a Current state process map which details the process in question as you found it when you started. Several graphical representations of the data you measured to confirm the state of the process as you found it in the analysis phase you should have utilize some methodology for statistically determining the root causes, in improve you determinedthe best solution (s) to the root causes and pehapsa pilot project with results that confirmed you fixed it. In the control phase you should have a control plan and a listing of lessons learned and perhaps future rocommendation on other issues affecting the process.

I am surprised that when you were taught the sixsigma methodology no one mentioned the storyboard.

Oh well..

David, I totally agree with this approach. 

I think the best way is to get to the point quickly; show the "what was" against the "new approach" and a couple of the Six Sigma tools used are good to demonstrate the value add of Six Sigma.  The key is if you are asked to do a 5 minute presentation, do just that.

As a MBB the best format is one that meets the certification guidelines as laid down by your MBB or certifying consultancy.  I.e if a Final Report contains all the elements that the MBB is looking for to certify you then that's great.  You may wish to lay it out in a logical storyboard format so that your presentation is understood by your 6 Sigma Champion and senior Execs alike - afterall you are fixing an issue and getting results.  Certification is just an outcome of undeerstanding & appplying appropriate tools, providing the right leadership and getting the result that will satisfy whom ever requested that you work the issue.

You are responding to an old posting.  Try a Google Boolean search for a SS final report format/template from the University of Michigan 's SS trining program.   It's a very good report template.   I have used it in the past, but don't have a copy of it at hand.   It also has a Powerpoint template for SS report presentations.

This is probably the longest message thread i have ever seen! i am surprised most people haven't been taught what the story board should look like ( some are certified BB's).

As some one pointed out earlier (last year) the key to an effective presentation is delivery; in terms of understanding what the sponsor/MBB is looking for. As long as you are able to convey what is the current performance (business case),how the current performance affects the process (problem Statement), what you would like to achieve (goal statement, could include monetary benefits), when you are going to get there (milestones for each stage of DMAIC) youi shouldn't be worried about it.

I would strongly recommend you to read "Package Delivery Optimization", an article published in "isixsigma Magazine" January/February 2005. The article walk you through each step of the DMAIC process. A total of 5 slides is all you need to do for your final report out. I would suggest that you suscribe to this Magazine, it will be $35 well spend.

Do you suppose there is a reason he never asked about using some of my Six Sigma work? Do you suppose I should offer to pay some kind of commission, maybe the highest in the industry, say 15 rupee for each usage to get things started? What do you think?

I shouldn’t tell this, but you’re obviously in pain and doubting the forum receptiveness of your wise counsel.

 

I routinely look for posters who have posted a personal email address and plead with them to respond to my ramblings positively.   When they say no or hold out for cash I have a special PayPal account that lets me move money into their bank accounts.  

 

It only costs me a few hundred dollars a month but the payback is in copying them and when someone comes into my office and asks if I’m screwing around the Internet again, I tersely wave the copies and say things like, “Screwing around???? …. Screwing around????…. I’m helping Phil and the rest of them keep our field centered!!!  Crap!!! What’s it take to get some respect around here????”   

 

I’ll probably be fired soon but then I know where our speed bumps and potholes are – and even who some of the speed bumps and potholes are, so maybe not.

 

But sorry,….you were in pain and my posting was all about me again.  

 

I will say however, I remember that time you gave some sound advice about Six Sigma analytics and remember thinking, “Crap, he does know his stuff!!!”

BTW, Daisy Duke say's hello. She remembers you from the early years growing up in Texas.

Excellent advice regarding PayPal. I guess that would beat "I'll be the guy wearing the iSixSigma tee shirt and the dredlocks at the ExxonMobil station out on Route 24, six miles from Totetissville at 6 p.m.Don't do anything stupid." which is how I usually settle up with people.

BTW, that sound advice about Six Sigma analytics came from Overnight Phil, whom I pay premium rates to use the screen name Phil from during he daylight hours. He'll appreciate your observation.

Come to think about it, it has been a while since I answered a post seriously.........Thanks Dr. Vinny, I'm cured.........

The word “crap” should only be used in the correct context – as within quotation marks for something said or a thought remembered.   It should never just be used directly within a sentence, that’s rude and shows poor form.  

 

A demonstration of sentences applying the rules for using the word “crap” within a technical forum would be:

 

  • Stan said to that little old lady asking whether or not Six Sigma would be a good career move for her grandson, “Crap, old lady… who let you out of the home???”     This is acceptable use by Darth describing a hypothetical Stan posting.

 

  • I was thinking to myself, “Gee Phil, should I post this crap or not, and then decided, I can’t let that GiMePro dweeb have the inane space all to himself....”  This is acceptable use by Phil posting a recent pre-posting introspection.

 

  • I don’t understand why the moderator lets Vinny’s crap be posted and stay once posted.    This is completely unacceptable use by Tronan as it is directly applied within a sentence without quotation mollification. 

 

Hope that helped.  Some of the forum posting idiosyncrasies go well past anything that you’d have learned in school, but it’s good that you keep asking and learning.

I believe you forgot one usage appropriate to our forum, the double-secret differentiated possessive punctuated pronoun.

  • In a brief interchange between Ms. Vinny and Vinny, during a  late spring, Sunday afternoon regarding habits, behavior and Six Sigma, "Ya big academic blowhard, you told me you were going to organize the crap in the garage, and don't give me any crap about doing a value stream analysis and flowchart on the project before getting started!!!!!!!!!!!!" This is acceptable when using the proper bold font and two usages of the word and at least 10 exclamation points in ending the statement, assuring proper reader involvement in the dynamics of the near miss of a large frying pan heading in Vinny's direction.

Dear ladies and gentlemen,

I'm totally new to the Sigma reporting, but I'm quite fascinated and confused about it's reliability. From what I'm reading, the report should contain the measurable gains, if it is a factory, I suppose it's possible to measure them, providing the whole plant will be changed into a "pilot plant" for the duratation of the report preparation. But how do you go about it in a consulting office where every approach should address individual needs of  a customer?

One more example comes to mind; in the early '80's Lee Iacoca laid off all the engineers at Chrysler, concentrating on rescuing of the corporation, one could have thought that  IT resulted in gains, as no engineers no cost, naturally not for long, but if short goal assumptions are made, driven by the short term profits, the results could be totally illogical.

How long on an average it takes to prepare a full Sigma report, and how acurate are the assumed gains in the reports are vs the reality ?
